Self-attested address affidavit is not considered as a legal document. It is mandatory to get it on the local stamp paper and to get the document notarised.

As per the Central Government circulars, notarized documents are not necessary only self-attested affidavits are sufficient. However, the concerned authorities may not accept your self-attested affidavit as such and you are required to create a public document. If you have no such documents then get a bank address in your current address or ask the postal authorities to issue such an address proof document.
