This affidavit is a declaration made by an individual that he/she has renounced his previous name. If an individual makes a false affidavit by stating the false statements and if found using his renounced name, then he shall be charged under section 191 and 193 of Indian Penal Code. This affidavit is needed to declare that an individual has discarded his previous name and has adopted a new name and now he shall be identified by his new name.
To make this affidavit legally enforceable, it is needed to be printed on a stamp paper of the appropriate value and be duly notarized. Also, publication of information related to the change of name in two Newspapers, one in local language and other in English language Newspaper should be done which is mandatory for government employees. This affidavit can be used If an individual has changed his/her original name to a new name and is no longer using the old name due to any reason.
